Structural Heart Interventions (TAVR, TMVR)

What are Structural Heart Interventions (TAVR, TMVR)?

Structural Heart Interventions are advanced, minimally invasive procedures performed to treat complex heart valve diseases without the need for conventional open-heart surgery. These interventions are particularly beneficial for patients who are considered high-risk for traditional surgical valve replacement.

TAVR (Transcatheter Aortic Valve Replacement) is used to treat severe aortic stenosis by replacing the narrowed aortic valve using a catheter-based approach.

TMVR (Transcatheter Mitral Valve Repair or Replacement) is performed to treat significant mitral valve regurgitation or stenosis using catheter-guided techniques.

During these procedures, a new valve or repair device is delivered through a blood vessel—typically via the groin—and positioned inside the diseased valve under advanced imaging guidance. These techniques restore proper blood flow, relieve symptoms, and improve heart function with shorter recovery times compared to open surgery.

Procedure Overview

Surgery Duration

1.5 to 3 Hours

Anesthesia

Local anesthesia with sedation or general anesthesia (depending on case complexity)

Hospital Stay

2–4 Days

Full Recovery Timeline

2–3 Weeks

Average Cost

$18,000 – $35,000*

*Cost varies depending on valve type, device selection, imaging requirements, hospital category, and patient condition.

Step-by-step process

How are TAVR and TMVR Performed?

The procedure is conducted in a hybrid cardiac catheterization laboratory equipped with advanced imaging systems.

Anesthesia and Preparation

The patient is given local anesthesia with sedation or general anesthesia for comfort and safety.

Vascular Access

A small incision is made, usually in the groin, and a catheter is inserted into a major blood vessel.

Valve Positioning

Using real-time imaging (fluoroscopy and echocardiography), the catheter is guided to the diseased valve.

Deployment of Valve or Repair Device

For TAVR, a collapsible replacement valve is expanded within the diseased aortic valve. For TMVR, a repair or replacement device is positioned to restore proper mitral valve function.

Function Assessment

Valve performance and blood flow are assessed immediately to ensure proper positioning and function.

Completion and Monitoring

The catheter is removed, and the patient is monitored in a cardiac care unit for stabilization and recovery.

These procedures significantly improve valve function and reduce symptoms such as breathlessness and fatigue.

Types of Structural Heart Interventions

The choice of intervention depends on the valve involved and patient risk profile.

TAVR (Transcatheter Aortic Valve Replacement)

Minimally invasive replacement of the aortic valve.

TMVR (Transcatheter Mitral Valve Repair)

Catheter-based repair of the mitral valve (e.g., edge-to-edge repair techniques).

Transcatheter Mitral Valve Replacement

Replacement of the mitral valve via catheter approach in selected patients.

Valve-in-Valve Procedure

Placement of a new valve within a previously implanted surgical valve.

Hybrid Structural Procedures

Combination approaches tailored for complex cases.

When are Structural Heart Interventions Recommended?

TAVR or TMVR may be advised in the following clinical situations:

  • Severe aortic stenosis causing breathlessness or chest discomfort
  • Significant mitral regurgitation not responding to medication
  • High surgical risk due to age or associated medical conditions
  • Heart failure symptoms related to valve disease
  • Degeneration of previously implanted surgical valves
  • Patients unsuitable for conventional open-heart surgery

Final suitability is determined after cardiology consultation, echocardiography, CT imaging, and comprehensive heart team evaluation.

Benefits of Structural Heart Interventions

  • Minimally Invasive Approach: No large chest incision required.
  • Shorter Hospital Stay: Faster recovery compared to open-heart surgery.
  • Improved Valve Function: Restores efficient blood flow through the heart.
  • Reduced Surgical Risk: Ideal for elderly or high-risk patients.
  • Enhanced Quality of Life: Rapid symptom relief and improved functional capacity.
